Florida Ranks Fifth Nationally in Measles Cases as Kindergarten Vaccination Rates Lag

Florida is among the five states with the highest number of reported measles cases in the United States this year, according to data released by the Centers for Disease Control and Prevention. State figures put Florida's 2026 total at 155 cases, against a national count of 2,566 confirmed cases as of August 13.
The state's kindergarten vaccination rate stands at 88.9 percent according to Florida Department of Health data, well below the 95 percent threshold that the World Health Organization identifies as necessary for herd immunity against measles. That gap is the central epidemiological fact behind Florida's case count.
Most of Florida's cases this year trace to a single outbreak that began in January in Collier County, centered on Ave Maria University. Measles is among the most transmissible pathogens known, and a single introduction into a community with sub threshold vaccination coverage can generate sustained transmission.
Why 95 percent matters
Herd immunity thresholds are a function of how contagious a disease is. Measles has a basic reproduction number substantially higher than most respiratory viruses, meaning one infected person in a fully susceptible population will infect a large number of others. The higher that number, the larger the share of a population that must be immune to interrupt chains of transmission.
For measles, that share is approximately 95 percent. At coverage below that level, the virus can circulate even though most individuals are protected, because the unvaccinated minority is sufficient to sustain transmission. This is why a rate of 88.9 percent, which sounds high in isolation, is epidemiologically inadequate for this particular disease.
The measles, mumps and rubella vaccine is administered in two doses, typically at 12 to 15 months and again at 4 to 6 years of age. Two doses are approximately 97 percent effective at preventing measles. The vaccine has been in widespread use for decades and its safety profile is among the most extensively studied in medicine.
Herd immunity is not an abstraction for the people it protects. Infants too young for their first dose, people undergoing chemotherapy or taking immunosuppressive medication, and individuals with certain immune conditions cannot be vaccinated or do not mount a full response. They depend entirely on the immunity of those around them.
Where Florida's coverage gaps are
State health department data shows substantial county level variation. Sarasota County has the lowest kindergarten vaccination rate in the state at 82.1 percent, while Calhoun County has the highest at 98.7 percent. That spread of more than 16 percentage points means the practical risk of an outbreak differs enormously depending on where a family lives.
County level averages themselves conceal school level variation. A district average can mask individual schools where coverage falls far below the county figure, and it is at the school and classroom level that transmission actually occurs. Health departments track school specific data precisely because that is the operative unit for outbreak risk.
Florida requires certain immunizations for school entry, documented on the Florida Certification of Immunization, with exemptions available for medical reasons and for religious objections. The share of students entering under exemption has increased in Florida over recent years, consistent with a national pattern.
The Collier County outbreak that drove much of this year's case count illustrates the dynamic. A concentrated community with lower than average coverage provided the conditions for sustained transmission after an initial introduction, which is the typical anatomy of a measles outbreak in a country where the disease is not endemic.
What measles does
Measles is frequently and mistakenly regarded as a mild childhood illness. It is not. The disease begins with fever, cough, runny nose and conjunctivitis, followed several days later by the characteristic rash. Patients are contagious from roughly four days before rash onset through four days after, which means transmission occurs before anyone knows measles is involved.
Complications are common. Approximately one in five unvaccinated people who contract measles in the United States is hospitalized. Roughly one in twenty children with measles develops pneumonia, which is the most frequent cause of measles death in young children. About one in a thousand develops encephalitis, which can result in permanent neurological damage.
Measles also produces a phenomenon known as immune amnesia, in which the infection degrades pre existing immune memory to other pathogens. Research has documented elevated susceptibility to other infections for a period of years following measles infection, an effect that is invisible in acute case counts.
A rare and uniformly fatal complication, subacute sclerosing panencephalitis, can develop years after apparent recovery. It is most associated with infection before the age of two.
The policy debate in Florida
Florida pediatricians have publicly pushed back against federal vaccine policy changes, arguing that they will worsen the measles situation in the state. Physician groups representing pediatric practices have been among the most vocal participants in the debate over vaccination requirements and messaging.
The argument from clinicians centers on the practical mechanics of vaccination uptake. School entry requirements are among the most effective tools public health has for maintaining coverage, because they establish a default and a schedule. Changes that weaken those requirements or that introduce ambiguity about recommendations tend to show up in coverage data within a year or two.
Advocates for broader exemption access argue that vaccination decisions belong to parents and that requirements attached to school enrollment impose coercive pressure. That position has gained ground in a number of states, and Florida's declining kindergarten coverage is consistent with a rising exemption rate.
The epidemiological reality is not contingent on the policy debate. At a coverage rate of 88.9 percent, Florida's population level protection against measles is below the threshold required to reliably prevent sustained transmission, and case counts reflect that.
What it means for Florida families
For families, the most useful step is verifying immunization status rather than assuming it. Records are held by the child's pediatrician and in Florida SHOTS, the state immunization registry, and county health departments can provide documentation. Children who are behind schedule can be caught up at any age.
Adults born after 1957 who do not have documentation of two doses or laboratory evidence of immunity may want to discuss the vaccine with a clinician. Adults born before 1957 are generally presumed immune from natural infection during the era when measles was universal.
Families traveling internationally should be aware that infants can receive an early dose at six months for travel, which does not replace the routine two dose schedule but provides protection during the trip. Measles remains endemic in many parts of the world, and imported cases are the origin of most United States outbreaks.
Anyone who develops fever with rash and has a known measles exposure should call ahead before going to a clinic or emergency department. Walking into a waiting room with measles exposes everyone present, and health facilities have isolation protocols they can activate if warned in advance.
Local impact across the state
Southwest Florida carries elevated exposure given the Collier County outbreak origin and Sarasota County's status as the lowest coverage county in the state. Families in that region face a materially higher baseline risk than families in high coverage counties.
Central Florida recorded its first case of the year earlier in 2026, and the region's high volume of domestic and international tourism creates continuous opportunity for importation. Theme park corridors bring together large numbers of people from many countries, which is the classic setting for an introduction event.
South Florida's international travel volume through Miami International Airport and the region's cruise terminals creates similar exposure. Northeast Florida and the Panhandle have generally reported fewer cases, though coverage rates vary by county there as well.
County health departments maintain immunization clinics and are the point of contact for families seeking vaccination outside a pediatric practice, including for uninsured children through the federal Vaccines for Children program.
How outbreaks are contained
Public health response to a measles case follows a defined protocol. The case is confirmed by laboratory testing, and health department staff conduct a contact investigation identifying everyone who shared airspace with the patient during the infectious period.
Measles is airborne and can remain infectious in a room for up to two hours after the patient has left, which makes contact tracing unusually broad. Exposure lists frequently include everyone present in a waiting room, a classroom or a store during a defined window.
Exposed individuals without immunity are offered post exposure prophylaxis, either the vaccine within 72 hours of exposure or immune globulin within six days. Those who cannot be protected are asked to quarantine for 21 days, which is the outer limit of the incubation period. Quarantine at that scale is disruptive, which is why containment is expensive relative to prevention.
The national picture
The United States eliminated endemic measles transmission in 2000, meaning the virus was no longer continuously circulating in the country. Cases since then have originated with importation from countries where measles remains endemic, followed by transmission in under vaccinated communities.
Elimination status depends on interrupting transmission chains within twelve months. Sustained transmission beyond that threshold would end the country's elimination status, a milestone that public health officials have identified as at risk given recent case counts.
With 2,566 confirmed cases nationally as of August 13, 2026, the current year ranks among the highest since elimination. Florida's contribution of 155 cases places it in the top five states, and the state's kindergarten coverage rate suggests continued vulnerability to additional outbreaks.
What is next
Case counts will continue to be reported by the Florida Department of Health and aggregated by the CDC. The pattern to watch is whether new outbreaks emerge in other counties rather than whether the total continues to climb, because a single sustained outbreak can account for a large share of a state's annual count.
The start of the school year is a period of elevated risk. Children returning to congregate settings after summer travel is the combination that has historically preceded outbreak clusters, and health departments increase surveillance accordingly.
Coverage data for the current kindergarten cohort will be compiled through the fall as districts complete enrollment documentation. Whether the 88.9 percent figure stabilizes, improves or declines further is the single most consequential number for Florida's measles trajectory over the next several years.
