Florida Ranks Among Top States for Measles Cases in a Record National Year

Florida is among the five states with the highest confirmed measles case counts in the United States this year, with roughly 141 cases reported in 2026. The figure sits inside a national total that federal surveillance put at approximately 3,134 confirmed cases as of September 3, a level that public health researchers have described as the highest in about 35 years.
Where Florida's cases came from
A substantial share of Florida's total traces back to Collier County in Southwest Florida, where an outbreak centered on Ave Maria University drove a surge earlier in the year. University settings are efficient amplifiers for measles because they combine dense residential living, shared dining and classroom spaces, and a population that moves frequently between the campus and surrounding communities.
Once introduced into a community with pockets of low vaccination coverage, measles spreads with a speed that distinguishes it from nearly every other communicable disease. The virus is airborne and can remain infectious in a room for up to two hours after an infected person has left. A single case in a susceptible population is estimated to produce a large number of secondary cases, which is why public health agencies treat even one confirmed case as an emergency requiring immediate contact tracing.
The Pan American Health Organization has separately warned of increased measles activity across the Americas. That regional context matters for Florida specifically, because the state's volume of international travel from the Caribbean and Latin America creates repeated opportunities for importation.
Why measles is different
Measles begins with what looks like an ordinary respiratory illness: high fever, cough, runny nose and red, watery eyes. Small white spots may appear inside the mouth. The characteristic rash typically follows three to five days after the first symptoms, starting at the hairline and spreading downward.
The infectious period begins roughly four days before the rash appears and continues about four days after. That means a person is contagious and moving through their normal routine before anyone, including the patient, knows measles is involved. It is the primary reason exposure notifications after a confirmed case often list grocery stores, urgent care waiting rooms, airports and places of worship.
Complications are not rare. Roughly one in five unvaccinated people who contract measles in the United States requires hospitalization. Pneumonia is the most common serious complication and the most frequent cause of measles deaths in young children. Encephalitis, an inflammation of the brain, occurs in about one of every thousand cases and can cause permanent disability. A rare and invariably fatal degenerative condition, subacute sclerosing panencephalitis, can develop years after apparent recovery.
The virus also produces a phenomenon researchers call immune amnesia, in which infection erases part of the immune system's existing memory of other pathogens. Children who recover from measles can be more vulnerable to other infections for a period afterward.
Vaccination coverage and the threshold that matters
The measles, mumps and rubella vaccine is highly effective. Two doses are estimated to be about 97 percent effective at preventing measles, and one dose about 93 percent effective. The standard schedule calls for the first dose between 12 and 15 months of age and the second between four and six years.
Because measles is so transmissible, the community protection threshold is unusually high. Epidemiologists generally place it around 95 percent two-dose coverage. Coverage below that level leaves enough susceptible individuals for sustained transmission, and coverage is not evenly distributed. A state or county average can look adequate while individual schools, congregations or neighborhoods sit well below the threshold.
Florida requires immunization for school entry, with medical exemptions and religious exemptions available. Exemption rates have risen in Florida as they have nationally, and the geographic clustering of exemptions is what turns an imported case into an outbreak.
Adults who are uncertain of their status have options. People born before 1957 are generally presumed immune through natural infection. Those vaccinated between 1963 and 1967 may have received an inactivated vaccine that was less effective and are sometimes advised to be revaccinated. A blood test can confirm immunity for anyone unsure, and an additional MMR dose is considered safe for someone who is already immune.
What it means for Florida families
The practical implications differ by household. Families with children on the standard schedule and complete two-dose coverage face low individual risk. Families with infants too young for the first dose, immunocompromised members, or unvaccinated children face substantially higher risk, and they depend on community coverage they do not control.
Parents planning international travel with infants should be aware that an early dose is available. Federal guidance permits an MMR dose for infants six through eleven months who are traveling internationally, though that early dose does not count toward the two-dose series and the child still needs both routine doses later.
Anyone who suspects measles is advised to call ahead before going to a clinic or emergency department rather than walking in. That allows the facility to arrange isolation and prevents exposure in a waiting room, which is one of the most common settings for secondary transmission.
Pressure on county health departments
Measles response is labor-intensive in a way that is easy to underestimate. A single confirmed case triggers interviews to establish the infectious period, reconstruction of the patient's movements, identification and notification of every potentially exposed contact, assessment of each contact's immunity status, and in some cases quarantine recommendations lasting up to 21 days.
Florida county health departments are managing that workload at the same time they are responding to an increase in locally acquired dengue in Hillsborough County and a confirmed local dengue case in Orange County. The two responses draw on overlapping staff, particularly epidemiologists and communications personnel, in departments whose surge capacity is finite.
School districts carry part of the burden as well. When a case is identified in a school setting, districts must work with health officials to review immunization records, identify susceptible students and communicate with families, all under significant time pressure.
The national picture
The approximately 3,134 confirmed cases reported nationally through early September represent a dramatic departure from the pattern that held for most of the past two decades. Measles was declared eliminated in the United States in 2000, meaning there was no continuous transmission for more than 12 months. Elimination status depends on maintaining high vaccination coverage and interrupting chains of transmission after importation.
Extended outbreaks lasting longer than a year in any part of the country put that status at risk. Public health officials have been explicit that the current national case count reflects declining childhood vaccination coverage rather than any change in the virus itself.
How an outbreak investigation runs
When a Florida county confirms a measles case, the response follows a sequence that public health agencies have refined over decades. The first step is laboratory confirmation, typically through a polymerase chain reaction test on a throat or nasal swab alongside serology.
Simultaneously, epidemiologists interview the patient to establish the infectious period, which runs from four days before rash onset through four days after, and to reconstruct every location the person visited and every person they encountered during that window.
Contacts are then assessed for immunity. Those with documented two-dose vaccination are generally considered protected. Those without documentation may be offered post-exposure prophylaxis, which can prevent or modify disease if administered quickly: the vaccine within 72 hours of exposure, or immune globulin within six days for certain high-risk individuals including infants, pregnant people and the immunocompromised.
Susceptible contacts who cannot receive prophylaxis may be asked to quarantine for up to 21 days, the outer limit of the incubation period. That request is voluntary in most circumstances but carries substantial disruption for families and workplaces.
The school exemption question
Florida requires specified immunizations for entry into public and private schools, child care facilities and family day care homes. Documentation is submitted on a state form, and schools are responsible for verifying compliance.
Two categories of exemption exist. A medical exemption requires certification from a physician licensed in Florida that the immunization is medically contraindicated. A religious exemption is available on request through the county health department, without a requirement to specify a particular belief.
Exemption rates in Florida have risen over the past decade, consistent with national trends, and the state-level figure conceals substantial variation. Individual schools, particularly some private and charter schools, report exemption rates far above the state average, and those institutions are where an introduced case can spread.
Public health officials describe the resulting geography as pockets of susceptibility. A state can maintain an overall coverage rate that appears adequate while containing communities where coverage sits well below the threshold required to prevent sustained transmission. Collier County's experience this year illustrated the pattern.
What districts and health departments can do about it is limited. Verification and follow-up on incomplete records are the routine tools. Changing exemption policy is a legislative question, and it is among the more politically contested subjects in Florida health policy.
What's next
Florida's case count will continue to be reported through Department of Health surveillance, and county-level detail is published as cases are confirmed. The fall and winter respiratory season typically increases the frequency of clinical encounters where measles can be mistaken for something else early on, which raises the value of clinician awareness.
The most consequential variable is vaccination coverage heading into the next school year. Districts and health departments have limited tools to change it quickly, and the effect of coverage decisions made now will show up in outbreak risk over the following several years rather than immediately.
For families, the actionable step is a records check. Confirming that every household member has documented two-dose MMR coverage, and addressing any gaps with a provider, is the one intervention available at the household level that reliably works.
